Elimination fails on $[\begin{matrix} a & 2 \\ a & a \end{matrix}]$ if $a = 2$ or $a = 0$ . (You could notice that the determinant $a^{2} - 2 a$ is zero for $a = 2$ and $a = 0$ .)
